Misconduct Reporting Form

This form is intended for BNU students and staff to report incidents of non-academic misconduct involving BNU students.

BNU Partner students may also use this form to report incidents of harassment or sexual misconduct directly to the University. All other types of misconduct should be reported through the partner institution’s own misconduct or disciplinary procedures.

Note: Anonymous reports or third party reports cannot be submitted through this form. If you wish to report anonymously, please use the  platform and select the â€˜Tell us anonymously’ option. You can also access support via the platform.

For more information on how the University handles anonymous reports, please refer to paragraph 54 of the Student Conduct Procedure.

Completing this form

  • Read the Student Conduct Procedure (and the Student Prevention of Harassment and Sexual Misconduct as applicable)
  • Please complete all sections fully with as much detail as possible.
  • Be clear and concise in describing your incidents
  • Let us know if you have any evidence to support the report

What happens next?

You will receive confirmation from the Resolution Team that the Misconduct Incident Report Form has been received.

Upon review of this Misconduct Reporting form, you may be asked for more information or invited to discuss the incident. You will be advised of next steps.

Where appropriate, prior to proceeding with formal action, the University may seek to resolve the matter through an Early Resolution Intervention. You may be asked whether you are amenable to this approach. If deemed suitable, an Early Resolution Invention will normally be completed within 10 working days of receiving this form.
